چکیده

M e t h o d s Design: Cohort study (Prospective Registry Evaluation Myocardial Infarction: Event and Recovery [PREMIER]). Setting: 19 academic centers, inner-city hospitals, single-payer systems, and nonuniversity hospitals in the United States. Patients: 2498 patients ≥ 18 years of age (mean age 60 y, 71% men, 79% white) who had biomarker evidence of myocardial necrosis within 24 hours of admission or who were transferred to the hospital within 24 hours of onset of symptoms and had acute MI (ischemic signs, symptoms for > 20 min, or electrocardiographic ST changes). Risk factors: Discontinuation of 1 or more of 3 evidence-based medications (aspirin, βblocker, or statins) at 1 month after MI. Results were adjusted for age, sex, race, educational level, marital status, smoking status, diabetes, hypercholesterolemia, hypertension, heart failure, coronary heart disease, revascularization during index hospitalization, type of MI, social support, avoidance of medication because of cost, and site of enrollment. Outcome: Death at 12 months.

Discontinuation of secondary prevention medication after myocardial infarction - the role of general practitioners and patients.

BACKGROUND Despite the significant benefits of secondary prevention (SP) medication after acute myocardial infarction (MI), evidence suggests that these medications are neither consistently prescribed nor appropriately adhered to by patients. The aim of this study was to investigate the role of general practitioners (GPs) and patients regarding discontinuation of SP medication after MI and reas...

QT-Dispersion as a potential marker in prognosis of acute myocardial infarction

Introduction: QT dispersion in a 12 lead ECG represents the heterogeneity of ventricular repolarization. The prolongation of QT-interval dispersion increases the risk of coronary heart disease, ventricular arrhythmia, and sudden cardiac death in patients with myocardial infarction. We aimed to investigate the prognostic value of QT-dispersion in patients with acute myocardial infarction. Meth...

Thienopyridine Use After Coronary Stenting in Low Income Patients Enrolled in Medicare Part D Receiving Maintenance Dialysis

BACKGROUND Coronary stenting in patients on dialysis has increased by nearly 50% over the past decade, despite heightened risks of associated stent thrombosis and bleeding relative to the general population. We examined clopidogrel, prasugrel or ticlopidine use after percutaneous coronary intervention (PCI) with stenting in patients on dialysis. We conducted 3-, 6-, and 12-month landmark analys...

Six-month versus 12-month dual antiplatelet therapy after implantation of drug-eluting stents: the Efficacy of Xience/Promus Versus Cypher to Reduce Late Loss After Stenting (EXCELLENT) randomized, multicenter study.

BACKGROUND The optimal duration of dual antiplatelet therapy (DAPT) after implantation of drug-eluting coronary stents remains undetermined. We aimed to test whether 6-month DAPT would be noninferior to 12-month DAPT after implantation of drug-eluting stents. METHODS AND RESULTS We randomly assigned 1443 patients undergoing implantation of drug-eluting stents to receive 6- or 12-month DAPT (i...

Potential Prognostic Significance of Decreased Serum Levels of TRAIL after Acute Myocardial Infarction

BACKGROUND Since soluble TRAIL exhibits anti-inflammatory and anti-atherosclerotic activities both in vitro and in animal models, this study was designed to assess the relationship between the serum levels of TRAIL and clinical outcomes in patients with acute myocardial infarction (AMI). METHODOLOGY/PRINCIPAL FINDINGS Levels of TRAIL were measured by ELISA in serial serum samples obtained fro...
